To activate your Ideal Dermatology Patient Portal, you will need to click on this link: idealderm.modmedapp.com. Your User Name is your email address and the password can be created the first time you log in.
In the Patient Portal, you can access your office visit notes from Ideal Dermatology, view billing statements, send a secure message to your provider, and update health history information, address, and phone number.

After activating your Patient Portal account on a computer browser, you can use our patient mobile app - APPatient™. To log into APPatient, you will need the User Name and Practice URL provided above. Please click on the appropriate link below to download the mobile app for your device.

Please note, if you need any pathology or lab result notes, please call our office as that is considered an outside record and you will not be able to access it in the Ideal Dermatology Patient Portal.
